Self-referrals
To ensure that one of our clinicians will be suitable to address your needs and goals, clients new to our clinic are asked to submit a Clinic Intake Form prior to their initial appointment. This information may be collected over the phone (403) 764-0771, sent to us via fax, (403) 764-0772, or submitted directly through our online client management system, which one of our staff members can set up for you.
You may also call us for a complimentary 10-minute phone consultation (403) 764-0771. If we are unable to provide the service that you are looking for, we will make suggestions as to who might be able to help.
